    • "Collector's Notes" (uncredited) are on a double-sided laid-in sheet.
    • This edition is a facsimile reprint of the 1989 Questar/Popular Library edition which corrected various typos of the first edition.
    • Locus1 dates this as April 1990, but this can not be confirmed because of the difficulty in determining the publication date of Easton Press editions.
