    • Illustrated limitation page appearing on the last page before the back loose endpaper bears the statement "The edition is limited to 300 signed and numbered copies. This is copy number" below which the number of the copy is printed (not handwritten) in a much larger font. This is followed by the signatures of Michael Kurland (the introducer), John Pelan (the editor and author of the afterword), and Jacob McMurray (the cover artist/designer).
    • "Published in 2018 by Centipede Press" stated on the copyright page. Delays in publication caused unsigned copies to be distributed in February 2019, and the signed copies in March 2019. Both Amazon and Barnes & Noble give the publication date as March 19, 2019 with a publisher's price of $75.00.
